This form is for NSU students to request Academic Accommodations related to a documented disability during their enrollment at NSU.
Housing Accommodations
This form is for NSU students to request housing accommodations based on a documented disability while residing in NSU residential housing. Please note that all students should also complete the parallel processes and meet the deadlines required in their housing portal and/or received via their NSU student email, including housing contracts, roommate selection process, roommate swap process, etc.
Semester Request for Services
Students who have currently approved accommodations with Student Disability Services use this form each semester to request notification of their approved accommodations to be sent to their professor(s).
All forms are available in an alternate format upon request.
